Greetings and Welcome!  

You may have some questions before your first appointment.  To assist you on your first visit to Dermal Dimensions™ Progressive Skin Therapy™ for the best skin rejuvenation, anti-aging, adult and teen acne treatments possible.

Forms

At your initial visit to Dermal Dimensions™ Progressive Skin Therapy™, you will be requested to complete a questionnaire with essential information about your skin's aging or acne condition.

Payment

Payment may be made in cash, personal or business checks, at the time of your appointment. Due to the extra expense, Credit Cards are not accepted for payment.  Checks returned due to insufficient funds will be subjected to a $35 fee.

Clinic Etiquette

The Dermal Dimensions™ Progressive Skin Therapy™ is located in beautiful Providence, Utah, to assist you in obtaining clear beautiful radiant skin...and soothe your mind as well. To maintain this environment we ask that when services are being performed for others, please speak softly.

Arrival Time: If this is your first visit, you are encouraged to arrive at least 10 minutes prior to your scheduled appointment time, to allow for filling out paperwork. If this is not your first visit, please arrive on time.

Late Arrival Time: If a late arrival is unavoidable, please call to notify when possible.  Please keep in mind that arriving late may regrettably limit your treatment time.  This office strives to stay on schedule to allow a pleasurable experience for all.  

Parking: Please park on the asphalt drive, or just before the drive (to the south).  Do not park within 15' of a mail box.  

Cancellation Policy and Procedure

I understand that life can be busy and accidents happen, and sometimes things are just beyond our control.  However please consider others that may be on a waiting list who would loudly take your point meant that you are unable to make. Last-minute cancellations may take up to 3 weeks to reschedule.    

A 24 hour cancellation policy is enforced. If less than a twenty-four hours notice is given, you will be charged a cancellation or No Show fee.

Gift Certificate: If you have made an appointment and are using a gift certificate, and you do not show or call 24 hours in advance to reschedule your appointment, Your gift certificate will be considered redeemed in full.

 Clinical Skin Therapy

Clinical skin  treatments are the personally selected for incredible results.

Shaving:  Generally, shaving is not recommended prior to body treatments or hair removal services. It is, however, recommended that men shave the evening or morning prior to a facial treatment.

Special Health Considerations: If you have health concerns or are pregnant, please be sure to make this known when booking your appointment, or upon arrival. Treatments most suited for you will be discussed.
